Who Qualifies for Supported Living?

Age
Members must be between 18 and 65 years old.
Diagnosis
A primary diagnosis of an intellectual disability or autism.
Waiver
Enrollment in the FIS Waiver or Community Living (CL) Waiver.

How It's Funded
Supported Living is paid for through Medicaid (DMAS) for members enrolled in the FIS or CL Waiver. Depending on the living arrangement, members may still be responsible for:

What is Supported Living?
Supported Living Services help adults with developmental disabilities live more independently in their own home, apartment, or a shared living arrangement, with personalized support for daily routines, transportation, medical appointments, community access, and life skills.
Who qualifies for Supported Living?
Members generally need to be 18 to 65 years old, have a primary diagnosis of an intellectual disability or autism, and be enrolled in the FIS Waiver or Community Living (CL) Waiver. Not sure if you qualify? Reach out and we'll help you check.
Is this the same as a Group Home?
No. Group Homes require the Community Living (CL) Waiver and offer more structured, residential support. Supported Living accepts either the FIS Waiver or the CL Waiver and is designed for individuals who want more independence, living in their own apartment, home, or a shared arrangement with support based on their needs.
What kind of support is included?
Support can include help with daily routines, transportation, medical appointments, community access, and life skills, all built around a person-centered plan.
Do I need to already have housing?
Not necessarily. Supported Living can take place in your own apartment or home, or in a shared living arrangement with a roommate we help match based on compatibility.
How is Supported Living paid for?
Supported Living is funded through Medicaid (DMAS) for members enrolled in the FIS or CL Waiver. Depending on the living arrangement, members may still be responsible for costs like utilities, internet, and rent, which can be income-based for apartment settings.
Does Smiles Like Yours provide transportation?
Yes. We provide transportation for Supported Living members to medical appointments, work, and community activities.
Can my loved one have a roommate?
Yes. Before placing anyone together, our team hosts activities and get-togethers to make sure personalities and routines are a good fit.
